Lifeways is Hiring an Assisted Outpatient Treatment (AOT) Monitor Near Jackson, MI

General SummaryUnder the direction of the Director, Crisis Services the Assisted Outpatient Treatment (AOT) monitor provides culturally competent and trauma-informed support and monitoring to all consumers. The AOT monitor gathers and distributes information across the systems of AOT, which includes court reporting, identification, tracking, and education. The AOT monitor provides liaison and monitoring services to LifeWays consumers, programs, and providers when an individual is identified as having been placed under court order. The AOT monitor ensures and facilitates the effective sharing of information between LifeWays and the systems of AOT to include Probate Court, hospitals, residential settings, and clinical programming leadership as assigned.
Essential Functions
  • Facilitates the exchange of information between LifeWays programs and providers.
  • Facilitates the exchange of information between LifeWays and hospitals related to consumers and programs.
  • Acts as liaison between LifeWays, Probate Court, residential facilities, crisis teams, and associated clinical programs or teams.
  • Assists in the development, implementation, and monitoring of LifeWays' AOT programming.
  • Assists in the development of and facilitation of AOT trainings across different systems.
  • Develops and maintains effective community partnerships including with court personnel, hospitals, law enforcement agencies, crisis services, network providers, and consumer advocates.
  • Conducts regular reporting on AOT program needs.
  • Collaborates with primary case holders (case managers, peers, therapists, etc.) to monitor treatment compliance.
  • Attends treatment team meetings, as indicated.
  • Monitors AOT expiration dates and reports to primary case holders on AOT needs.
  • Ensures compliance with all applicable grant requirements.
  • Ensures compliance with all applicable mental health laws pertaining to treatment order.
  • Provides training as needed and requested.
  • Maintains regular and predictable attendance.
  • All other duties as assigned.
Required Education/Certification/Licensure
  • Bachelor's Degree in Human Services related field.
  • Must possess a valid U.S motor vehicle operator's license, and successfully complete a motor vehicle record check.
Preferred Education/Certification/Licensure
  • Bachelor's Degree in Criminal Justice, Nursing, or Social Work
  • Applicable licensure in the State of Michigan.
