LifeStream is a behavioral health and social services organization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando counties.

JOB SUMMARY: This is a highly responsible position that provides coordination between the Access Center and the Inpatient Services programs. This position works with the Director of Acute Care Services, Hotline Counselors, Evaluators, Recovery Specialists, Activities team and the RM/QI Director in developing, implementing, and monitoring organizational policies, practices and procedures for the programs listed. This position performs quality reviews, facilitates Treatment Team and ensures all court work is accurate and submitted timely.

  • Prepare activity reports to inform management of the status and implementation plans of programs, services, and quality initiatives 
  • Establish objectives and evaluate operational criteria for programs they coordinate with 
  • Develop instructional materials and conduct in-services and community-based educational programs 
  • Develop or expand and implement programs or health services that promote rehabilitation and community health 
  • Consult with medical, business, and community groups to discuss service problems, respond to community needs, enhance public relations, coordinate activities and plans, and promote programs 
  • The ability to comply with the regulations requiring a workplace of dignity and respect and equal employment opportunity compliance
  • The ability to work collaboratively with others and to accept constructive criticism from supervisors and managers

EDUCATION & EXPERIENCE

  • Education and Experience: Bachelor’s Degree in Social Services or 4 (four) years of experience, with at least 2 (two) years of that experience in mental health.
  • Driving Requirements:
    • Must be Over the age of 21 required
    • Minimum of 5-years driving experience required
    • Possess a Valid FL Driver’s license (within 30 days of hire if currently do not possess)
    • Possess a continuously valid US Driver’s license for the past three years, from date of issue required
    • Safe Driving record required
