LPN - FACT 050

JOB SUMMARY: LPNs of the Florida Assertive Community Treatment (FACT) multidisciplinary team will be responsible for administering injections, delivering oral medications, scheduling, and tracking lab results, and collaborating with doctor’s offices and pharmacies to ensure consumer medications are up to date. LPNs will also assist consumers in attending psychiatric appointments, healthcare appointments and lab appointments. FACT LPNs will be responsible for assisting consumers in increasing social skills, improving level of functioning and gaining insight into their illness including: offering supportive counseling, administering assessments and participating in treatment team meetings while collaborating with other FACT staff. This is a Field Based position meaning 75% of the workday will be visiting consumers in the field providing services.

Qualifications:

• Graduation from an accredited school of nursing meeting the minimum standards of the Florida State Board of Nursing as a Licensed Practical Nurse
• Minimum of 1 year of experience with individuals with severe and persistent mental illness
• Must be able to pass a Level II Background Screening
• Satisfactory Driving Record while possessing a Valid FL Driver's License
