Job Summary: Are you passionate about serving others and your community? LifeSpan Resources is looking for compassionate and skilled professionals to join our team! The Case Manager is actively involved within our ever-growing community! We are an agency that provides on-going waiver case management services located in Southern Indiana. We strive to promote independent living for people of all ages! Our mission is to provide independence and quality of life for all persons through advocacy, education, and client centered services! We strive to be active in the community whether it be through our Senior Games, Voice and Vino, or Meals to Go. The Case Manager will complete all components of on-going case management for waiver and non-waiver programs. The goal is to provide access to quality home and community-based services accurately, consistently, and efficiently. The Case Manager will be constantly engaged within the community and be involved in our activities! Job Responsibilities:
  • Establish and sustain interpersonal relationships.
  • Ability to empathize and understand individual behaviors.
  • Assist with Medicaid application process.
  • Comply with FSSA/IDA manuals, as well as Federal, State and local laws.
  • Aid in long term support planning and goal setting.
  • Determines program eligibility by utilizing screening and assessment tools.
  • Knowledge of State & Federal laws.
  • Execute other assignments as requested.
Qualifications
  • An individual continuously employed as a Care Manager by an Area Agency on Aging (AAA) since June 30, 2018; or 
  • A registered nurse, a licensed practical nurse, or an associate degree in nursing with at least one year of experience serving the program population; or 
  • A Bachelor's Degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services; or 
  • A Bachelor's Degree in any field with a minimum of two years full-time, direct service experience with older adults or person with disabilities (this experience includes assessment, care plan development, and monitoring); or 
  • A Master's Degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services; or 
  • An associate degree in any field with a minimum four years full-time, direct service experience with older adults or persons with disabilities (this experience includes assessment, care plan development, and monitoring). 
  • Ability to lift up to 25 lbs. on an occasional basis.
