JOB TITLE: Recovery and Wellness Counselor
DEPARTMENT: Elder Abuse
PROGRAM: Recovery and Wellness-Livingston County
SUPERVISOR: Recovery and Wellness Program Manager
STATUS: Non-exempt
Hourly Rate: $21.50 to $22.50
SUMMARY (Full or Part Time)
Provides addiction counseling and case management for clients of the Geriatric Addictions Program and their caregivers.
DUTIES AND RESPONSIBILITIES
Program operations
1.0 Provides informal assessment and referral for the Geriatric Addictions Program.
1.1 Provides comprehensive chemical dependency evaluations in conjunction with assessments of the medical/social needs of the elderly client in their home environment.
1.2 Develops comprehensive service plans.
1.3 Provides counseling, advocacy, and case management services to participants in the Geriatric Addictions Program.
1.4 Maintains professional standards, including accurate record/data keeping
1.5 Facilitates and/or initiates collaboration with local chemical dependency treatment facilities, mental health providers, law enforcement, medical providers and judicial entities as appropriate.
1.6 Prepares and submits reports as required in a timely manner.
1.7Develops program education/outreach materials and training modules under the direction of the Vice President for Program for the Geriatric Addictions Program.
1.8 Conducts community and professional educational/outreach presentations on topics relating to the identification of geriatric addiction, aging, mental wellness and caregiver concerns.
Finance and budget
2.1 Operates within the constraints of the allowed budget specifications.
Personnel/employee relations
3.1 Maintains a spirit of cooperation and team work within the Geriatric Addictions Program, the Elder Abuse Program and within the Agency.
Other
4.1 Attends appropriate workshops and conferences for professional/personal enrichment and skill building.
4.2 Participates in outreach for public awareness regarding the issue of geriatric addiction.
4.3 Maintains knowledge of community resources.
4.4 Maintains knowledge of mental health services for the elderly.
4.5 Maintain professional credentials and licenses.
5. Other duties as assigned by supervisor
Requirements
QUALIFICATIONS
EDUCATION: MSW, masters, preferred or BSW or bachelor’s degree in allied field required with NYS-CASAC
EXPERIENCE: Two years in chemical dependency counseling with experience in geriatrics.
