Title: Intake Therapist

Worksite: Warren County Service Center

County: Warren

Hours Status: Full-Time

Pay Range:  $21.63 hourly

Job Responsibilities include:

· Perform clinical evaluations and assessments, which will include diagnosis.

· Complete suicide screenings and safety plans when necessary

· Intake, screening, referrals, and crisis intervention services

· Provide coverage for Individual, family and group therapy to address various emotional disorders

and mental illness when needed.

· Prepare treatment plans, with goals and objectives.

· Work as part of a multi-disciplinary team with emphasis on goal oriented, evidenced based

therapy.

· Clinician will need to communicate effectively to ensure timely follow-ups and referrals.

· Complete all required documentation per LifeSkills policy.

· Other duties as assigned

Requirements for this position include:

Must have a minimum of a Master’s degree in Social Work, Psychology, Professional Counseling, or similar degree from an accredited university. Must be licensed or have the ability to become licensed to provide mental health services in the state of Kentucky. A Qualified Mental Health Professional - fully licensed clinician is preferred. No evidence of past malpractice claims or any State Board sanctions. Must pass a legally required Criminal Record Check and a Sexual Offender Registry check. Must have a valid driver’s license, reliable transportation, and present proof of automobile insurance.
