LifeSTEPS is a 501(c)(3) nonprofit charitable organization. It was founded in 1996 and currently has over 400 partnered affordable housing properties all over the state of California. We serve several regions from Northern CA, Sacramento, Bay Area, South Bay, Central Valley, LA Valley, Inland Empire, and LA South Region. We work with 100,000 residents all over the state and were on a mission to provide effective educational and supportive services to maximize the strengths of individuals and build resilient communities.

About the Position
The Community Health Worker (CHW) is responsible for helping members and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Case Managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of members and their family. CHW provides social support and informal counseling, advocates for individuals and community health needs.
