Responsibilities

• Complete initial and ongoing assessments of all individuals of 18-person caseload 

• Assess patients’ conditions as needed

• Send chart observations, and perform evaluation tasks including vital signs and medication reviews

• Assist PA with monthly clinics on site

• Oversee medication administration by unlicensed personnel

• Assist with quarterly Psychiatric clinic

• Ensure appropriate medical services are provided for all IDD individuals

• Assist with medical appointments

• Provide effective communication to patients/clients, their family members, team members, and other health care professionals

• Complete Workers Compensation paperwork and follow up as needed

Qualifications

• North Caolina state license as a LPN

• Proof of current CPR certification 

• Compassionate, friendly demeanor 

• Excellent communication skills

• Current driver’s license and reliable transportation 

• Ability to work a flexible schedule

• On call availability
