BACHELOR'S DEGREE IN MENTAL HEALTH FIELD REQUIRED

1. Obtain certification or license as required by the Department of Mental Health and maintain Certification/License in good standing.

2. Facilitate, engage, and assemble the family/youth team at a minimum every thirty days.

3. Facilitate the creation of the plan of care, which includes a plan anticipating, preventing and managing crisis, within the youth and family team meeting.

4. Work with the team in identifying providers of services and other community resources to meet family/youth needs.

5. Make necessary referrals for family/youth.

6. Maintain current documentation as required by state and local standards.

7. Serve as liaison between family/youth, home, school, and community resources such as family service agencies, child guidance clinics, courts, courts, protective services, doctors, local MAP teams and clergy members.

8. Lead the team to discuss and ensure the supports and services the family/youth are receiving continue to meet caregiver and youth's needs.

9. Display punctuality while performing all job duties.

10. Educate new team members about wraparound process.

11. Provide assistance and support in emergency and/or crisis situations as needed.
