We're currently seeking a Full Time Vocational/Residential Case Manager to work in our Transitional Housing Program for homeless individuals. The primary goal of this program is to provide an environment and level of services which will enable residents to attain and maintain safe and affordable permanent housing of their choice. Our employees have passion around the services they provide and our corporate values of excellence and staff support. The Vocational Case Manager will work to screen for and address any barriers that may have a negative impact on an individual’s ability to obtain and maintain employment. Vocational/Residential Case Manager Job Responsibilities: Completes Intake Assessments for all residents of DePalmer House.Participates in the development of an individualized service plan for assigned clients; coordinates and facilitates the delivery of services in accordance with service plan.Understands and applies case management principles to increase DePalmer House resident self-sufficiency and transition from unemployment to competitive employment.Responds effectively to client emergencies, which may require flexibility of both time and areas of assignment.Establishes relationships with and utilizes community service providers and employers for a broad range of client needs, including referral to housing, medical care, addiction treatment, mental health counseling, educational and vocational services.Completes all required documentation, in timelines in accordance with program standards.Demonstrates the values of diversity, equity and belonging; foster an inclusive environment that facilitates diversity. Vocational/Residential Case Manager Qualifications: Bachelors/Associates Degree and two (2) years’ related experience in vocational services. Must possess a valid New York State Driver’s License and have access to reliable transportation.
