**Overview:**

We are seeking compassionate individuals to join our team as Home Health NURSE/LPN. As a Home Health NURSE, you will provide essential care and support to individuals in their homes, assisting them with daily activities and ensuring their well-being.

**Responsibilities:**

- Assist clients with personal care tasks, checking vitals, helping with medicine, skin checks

- Provide medication reminders and assist with medication management

- Support clients with mobility assistance and transfers

- Monitor and report changes in client's health status to the appropriate healthcare professionals

- Offer companionship and emotional support to clients

**Qualifications:**

- Prior experience as a Home Health Nurse or LPN

- Knowledge of basic medical terminology and procedures

- Ability to follow care plans and communicate effectively with clients and healthcare providers

- Understanding of infection control practices and safety measures

- Compassionate attitude and strong interpersonal skills

**Skills:**

- Botulinum toxin treatment

- Physiology knowledge

- DME (Durable Medical Equipment)

- Discharge planning

- Pain management

- Memory care

- Infant care

- Experience in an Emergency Room (ER) setting is a plus

- Familiarity with Assisted Living facilities

- Ability to phlebotomize
