Job Summary:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and providing comprehensive care to patients, ensuring their physical, emotional, and social needs are met. POSITION: Case Manager RN

Shift: 11:00 AM to 07:30 PM / Variable

Rate: $71 - $75 /hour

LOCATION: 12254 Bellflower Blvd. Los Angeles Downey CA USA 90242

• Remote/computer work in a KP Office in Downey
• Case manager position that will be responsible for care coordination for members on Advanced Medical Care at Home Program (AMCAH). Duties involve enrolling members, conducting virtual visits alongside MDs, medication coordination with Pharmacy, lab ordering, discharging of members, referrals to any specialty programs and Post hospital Appointments.

Required Qualifications:

• 1 YR of Home health Care Management Experience from the last 3years.

REQUIREMENTS:

• State License/Certification
• Basic Life Support (BLS) AHA
• Must be COVID-19 vaccinated and boosted
