INTRODUCTION

POSITION SUMMARY
A qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.
ESSENTIAL FUNCTIONS

- Must be in good physical condition as the position requires the ability to stand on one’s feet throughout the day.
- Must be able to complete performance requirements independently while using good judgment as well as have the ability to harmoniously work with others.

- Must be able to read, write and understand the English language.
QUALIFICATIONS
- Graduate of an approved school of professional Nursing.
- Current licensure in good standing with the state in which the LPN will be practicing.
- Prefer at least one (1) year of experience as a professional nurse (hospital/nursing home/home health).
- Computer knowledge.
- Ability to work independently with minimal supervision.
- Exceptional communication skills with focus on quality patient care and customer satisfaction.
- Is flexible, cooperative, team player in fulfilling all obligations.
RESPONSIBILITIES
- Provides skilled nursing care as outlined in the nursing care plan to include the following:
- Nursing services, treatments and preventative procedures requiring substantial specialized skill as ordered by the physician.
- Initiating preventative and rehabilitative nursing procedures as appropriate for the client’s care and safety.
- Observing signs and symptoms and reports reactions to treatments, including drugs and changes in the client’s physical or emotional condition to the RN as necessary.
- Teaching the client and caregivers regarding the nursing care needs and other related problems of the client at home.
Job Duties:
- Submits all documentation accurately and in a timely fashion per policy.
- Participates in case conferences.
- Discusses with the supervisor problems concerning the clients and how they may be best handled.
- Discusses with the supervisor the need for involvement of other members of the health team such as the home health aide, physical therapist, speech therapist, occupational therapist, social worker, etc.
- Follows the nursing care plan for each assigned client.
- Coordinates care with the interdisciplinary team in order to provide continuity of care .
- Participates in staff development meetings.
- Continually strives to improve his/her nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual research and reading.

- Supportive of individual and company performance improvement initiatives.
- Maintain confidentiality with employee and company sensitive information.
- Comply with accepted professional standards and practices.
- Perform other duties as assigned by the Supervisor.
