DUTIES/RESPONSIBILITIES: Care Management for High-Risk Pregnancies and Care Management for At Risk Children provides formal case management services to eligible women during and after pregnancy, and to newborns, infants and children up to 5 years of age. Educates and counsels women enrolled in the Care Management for High-Risk Pregnant Women (CMHRP) Program. Educates and counsels parents of children enrolled in Care Management for At-Risk Children (CMARC). This position requires flexibility, good communication, computer skills, and making home visits.
MINIMUM REQUIREMENTS: Bachelor’s degree in social work from an appropriately accredited institution; bachelor’s degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor’s degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
Applicant must have a valid NC driver’s license. Preference will be given to those who have worked with pregnant women and children.
