Job Description

DeafLink Case Coordinator

Reports To: Director of DeafLink

Hours of Work: Hourly, non-exempt – part time

Approved by: President/CEO

POSITION SUMMARY: Incumbent provides case coordination services for consumers, who are Deaf or hard of hearing (D/HH), primarily in northeast Indiana.

EXPECTED RESULTS: The efforts of this position should result in:

1. Providing one “Breaking Down Barriers” event in partnership with the Inclusion Institute each quarter. (Example: Taxes/Creating a Will)

2. Attend two meetings/Deaf Community events per quarter.

3. Operating a table at a minimum of two Deaf specific outreach events each year. One in the spring/summer and one in the fall/winter. (Example: PTCO day/Deaf Festival)

4. Submit billing to receive the maximum amount of funding available through the State of Indiana case management contract. A minimum of 10 direct service hours each week should be provided to clients.

5. Filling out additional paperwork, as required, for The League’s ongoing documentation of case management services.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

Service Delivery

·Assist consumers in identifying goals that they would like to accomplish.

·Provide case coordination services for consumers in the DeafLink service area in accordance with identified consumer goals and/or written plans.

·Assist consumers in accessing needed community services, supports and resources.

·Teach consumers how to resolve problems or issues independently and advocate for their own needs.

·Monitor consumer progress relating to consumer goals and/or written plans.

·Provide work incentives and state and federal benefits training.

Public Relations/Marketing

·Inform consumers, businesses, government entities, and social service entities about the services provided through DeafLink and their benefits.

·Maintain positive relationships with the deaf community, interpreters, co-workers, and other professionals.

·Attending a minimum of one NIDAC meeting and one other meeting or Deaf Community event each quarter. It is highly encouraged to attend all the NIDAC meetings (once monthly)

·Attend a minimum of two D/HH specific outreach events with informational materials each year.

Administrative and Clerical

·Submit accurate and complete weekly time sheets that are consistent with information contained in case coordination forms.

·Develop reports and provide statistical data as requested by the League administrators.

·Provide policy and procedure recommendations relating to the DeafLink program, as needed.

·Maintain an outreach contact list from targeted events to grow DeafLink reach.

Financial

·Provide the League Finance department and DeafLink Director with information needed to send billing invoices to case coordination funding sources monthly and as requested.

Planning

·Plan and coordinate “Breaking Down Barriers” events (in partnership with the Inclusion Institute) to target areas of need to promote self-help and self-advocacy.

Personnel

·Incumbent is not directly responsible for any League personnel activities.

SUPERVISORY RESPONSIBILITIES: Incumbent does not supervise any paid staff.

QUALIFICATIONS, REQUIREMENTS and DEMANDS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative, and include the knowledge, skills, and/or abilities required. EDUCATION and/or EXPERIENCE: Minimum requirement of a high school diploma or GED. BA/BS in Health and Human Services; Psychology, Counseling, Bachelor of Social Work and/or Master of Social Work (M.S.W.) preferred. Extensive knowledge of the Deaf culture is required. Knowledge of, or ability to learn federal, state, and local civil rights provisions and support systems available to people with disabilities required. Prefer a qualified person with a disability.

LANGUAGE SKILLS: Excellent receptive and expressive sign language skills in American Sign Language. Ability to speak effectively before groups of customers, DeafLink business customers, and community groups. Ability to read and interpret written documents such as professional publications, policy and procedure manuals, and business and professional letters. Ability to write routine reports and correspondence in exact English. Prefer a qualified person who can translate American Sign Language, exact English and manual codes for English into written English.

MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.

REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables.

PHYSICAL DEMANDS: While performing the essential functions of this job the employee is required to:

1. Regularly use fingers, hands, arms, facial expressions and “body language” to communicate.

2. Frequently sit, stand, and walk.

3. Occasionally lift and/or move up to 20 pounds.

EQUIPMENT OPERATION REQUIREMENTS: Ability to independently operate TTY and video phone to communicate with consumers. Ability to independently operate or learn to independently operate League office equipment such as fax and copy machines to perform job related duties and assignments. Ability to independently operate or learn to independently operate the League’s computer network and software to perform job related duties and assignments.

TRAVEL REQUIREMENTS: Ability to travel independently throughout the DeafLink service region. Occasionally in an interstate region. Must have access to reliable transportation while working.

WORK ENVIRONMENT: While performing the duties of this job, the employee is occasionally exposed to fumes or airborne particles and outside weather conditions while traveling to consumer service locations. The noise level while traveling is usually moderate. Noise levels in the League office and consumer service environments are usually low.

WORK SCHEDULE DEMANDS: Weekdays - Ability to work during the day and/or evening on a scheduled basis. Weekends - Ability to work during the day and evenings occasionally (one Saturday per month).
