MDS Coordinator Skilled Nursing Facility
LeaderStat has a new opportunity for a permanent MDS Coordinator (RN or LPN) in Kalamazoo, Michigan. We are looking for a driven, experienced, and team oriented MDS nurse who wants to be part of a successful team!
Job Duties
- Assessing resident needs and coordinating individualized care plans based on assessment results
- Collaborate with all departments to ensure that care plan guidelines are followed
- Ability to accurately determine PDPM categories, HIPPS codes
- Ensuring that the nursing facility is receiving accurate and maximum reimbursement from Medicare and Medicaid Qualifications
- In-depth working knowledge of RAI user manual, as well as PDPM / HIPPS
- Active RN license is preferred, will consider a strong LPN with MDS experience
- Knowledge of current state, federal, and Centers for Medicare & Medicaid Services (CMS) regulations
- Exceptional attention to detail
- Strong computer literacy and competency skills
- Up-to-date clinical compliance
