A DAY IN THE LIFE OF A CARE COORDINATOR

In this care coordinator position, you provide assessment and treatment planning services to individuals with a serious mental illness. Following a strengths-based and trauma-informed approach, you focus on supporting individuals with their treatment goals. Because meaningful and measurable goals are essential for individuals receiving services, that is always your priority.

You also assist clients as needed with case management needs and crisis intervention services. Conscientiously, you monitor and evaluate your clients' progress towards specific goals around housing, employment, symptom management, and harm reduction. Making a difference one life at a time fills you with a deep sense of satisfaction, which is why you're perfect in this social services role!

QUALIFICATIONS FOR A CARE COORDINATOR

This position will meet the following requirements:

  • Master's degree in mental health, social services, or a related field
  • Certification as a qualified mental health professional (QMHP) through the Mental Health and Addiction Certification Board of Oregon

Additionally, candidates are required to fulfill requirements set by the Laurel Hill Center, including:

  • Valid Oregon driver's license
  • Willingness to undergo a background check and drug screening upon hire
  • Full vaccination against COVID-19

WORK SCHEDULE FOR A CARE COORDINATOR

This full-time behavioral health position works five 8-hour days or four 10-hour days. Hours are between 8:00 am - 5:00 pm. This position is hybrid with some in-office days and some remote-work days.
