Overview

The Larimer County Department of Health and Environment (LCDHE) is seeking a Nursing Supervisor for the Nurse Family Partnership (NFP) home visitation program!

NFP is a research and evidenced based program that has been demonstrated to significantly improve critical outcomes for at-risk, first-time parents. The NFP Supervisor effectively uses principles of supervision, reflective practice, and staff development to provide support and oversight to a team of nurse home visitors as well as build and model program delivery. Another important function of the NFP supervisor is to lead agency structure to ensure that the NFP program is implemented in accordance with established program guidelines, contract requirements, and departmental values.

The typical schedule for this position is 40 hours per week, Monday through Friday with required weekly supervisor and team meetings. Some hybrid work may be possible, and occasionally some evenings and weekend work is possible.

This position is limited term. Individuals hired into limited term positions are eligible for County benefits, however, the position is limited in duration dependent upon available funding, business need or signed contract or agreement. Nurse Family Partnership is a long-standing grant funded program.

Responsibilities

Specifically this position:

  • Provides supervision and evaluation of NFP home visitors and administrative support staff.
  • Ensures that supervision of NFP home visitors uses principles and concepts of the parallel process and reflective practice.
  • Effectively assists home visitors in analyzing complex problems and case situations.
  • Mentors and builds confidence and skill of nurse home visitors.
  • Assigns client caseload and monitors nurse home visitors ability to build and manage caseload.
  • Supervises routine personnel matters including recruiting, interviewing, hiring, training, assigning work, scheduling work hours, granting leave, appraising performance, disciplinary actions, and submitting such records and reports as required by County policies and procedures.
  • Develops and maintains community referral sources, runs quarterly Perinatal Coalition and referral handling.
  • Uses agency policies and procedures and develops needed policy and procedure to guide implementation. Monitors program efficiency and effectiveness.
  • Oversees and ensures timely and accurate data input and utilization of web-based reports.
  • Ensures implementation of Quality Improvement strategies and complete and timely documentation by home visitors.
  • Plans and expends budget to ensure compliance with fidelity to NFP model and contract agreements.
  • Participates as an active member on the Department’s Manager Team.
  • Responds to questions and concerns from clients, community agencies, and the public.
  • Provides direct health care and instruction to clients when needed.
  • May participate in emergency preparedness and response activities.

Qualifications

All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.

  • Three (3) years of increasingly responsible experience, including one (1) year of supervisory experience required.
  • Bachelor's Degree in Nursing from a school accredited by the CCNE (Commission on Collegiate Nursing Education) or NLNAC (National League for Nursing Accrediting Commission) required.
  • Possession of a valid Driver's License by date of hire required.
  • Licensed to practice as a registered nurse (RN) in the State of Colorado required.
  • Maintain continuing education hours annually, or as required by the Department, and obtain certificates/training necessary for specific programs required.
