Mission for Position

The Community Outreach Coordinator serves as a frontline liaison between LHS and the communities we serve. This position is responsible for serving as a link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. This role will serve as a bridge between the community and the health care, government and social service systems. This person will build a trusting relationship with individuals and families in our community and will be a key member of our patient-centered health teams and community collaborators.

Duties & Responsibilities

Priority 1: Improving Access to Resources and Care

  • Provide direct outreach to patients to increase utilization of primary and preventative health care
  • Link patients to community resources and assist in navigating internal and external systems
  • Refer and link to preventative services through health screening and healthcare information
  • Enhance and expand clinic-based screenings, care coordination, and referral processes to connect internal programs and community-based initiatives to patients who demonstrate social needs and challenges
  • Engage in facilitating internal nutrition insecurity programs
  • Be a spokesperson/advocate for patients when needed

Priority 2: Bridge the gaps between healthcare and communities / social service systems

  • Increase access to care by providing culturally appropriate health information to patients, community members including immunization education
  • Navigate individuals with the knowledge, skills and expanded resources to obtain healthcare at the right time and right place
  • Enhance LHS care delivery by aiding communication between healthcare providers, patient and community via enhanced cultural practices.
  • Align and promote health and well-being activities as part of infrastructure projects and with community sectors

Priority 3: Building and expanding community capacity for health improvement

  • Develop and maintain strong relationships with community partners, organizations and the key populations dedicated to serve
  • Serve as key point of contact for community partners related to collaborative health improvement strategies
  • Coordinate with Marketing and Communications department to create meaningful messaging and marketing collateral to support health improvement initiative(s) for targeted population(s).

Qualifications

  1. Community Health Worker certification preferred.
  2. Experience with community health, public health or related field.
  3. Must have strong written and oral communication skills, including preparation and report presentation.
  4. Knowledge of personal computers, PC applications, MS Word, Excel, PowerPoint and Project.
  5. Effectively communicate with community members, partners and stakeholders.
  6. Communicating health information using culturally appropriate terms and concepts
