Lakeview Supported Living Services is Hiring a Client Services Manager Near Madison, WI

Description:

The Client Services Manager, operating as part of the Client Services Team, ensures the delivery of high quality services for clients of Lakeview Supported Living Services. With direction and support from the Director of Client Services, the CSM identifies, develops, coordinates, monitors and participates in the residential and community services for clients. The responsibilities of the Client Services Manager include, but are not limited to:

  • Developing and maintaining a strong rapport with each Lakeview client
  • Working a minimum number of direct support hours with clients (daytime, evening, weekend or overnight).
  • Managing Caregivers to maintain Lakeview's expectations for high quaility support
  • Developing, implementing, monitoring, reviewing and updating personalized Support Plans for clients
  • Maintaining an expert level understanding of the Support Plans for clients
  • Working with the DCS to coordinate and maintain Client and Support Staff schedules
  • Attending internal and external team meetings for clients
  • Collaborating and communicating professionally with internal and external teams and guardians for clients
  • Acting as the primary point of contact for internal and external teams and guardians for clients
  • Facilitating Meet and Greets (introductions) between clients and potential Support Staff
  • Delegating responsibilities and supervising the work of Support Staff working with clients
  • Providing initial and ongoing training for both new and existing Support Staff per the Support Plans
  • Working with the DCS to track and monitor timekeeping records, tasks, wellness questions and shift notes for clients
  • Evaluating the performance of Support Staff
  • Understanding the high level support needs of all clients within the LSLS Network
  • Answering phone calls and responding in person as the On-Call Manager for the entire network of clients during scheduled/assigned timeframes.
  • Participating in the Interview, Training and Development, Performance Review and Termination processes of the agency.
  • Filling in on open or available shifts with all clients in the LSLS Network as needed.

Minimum Qualifications:

  • Associates Degree or Bachelor's Degree
  • At least 2 years of Health Care, Social Services or Direct Support experience or related college level courses.
  • Ability to successfully pass a Caregiver Background Check.
  • A valid driver’s license with personal transportation that is reliable and insured.
  • A smartphone with suitable voice and data plan for daily calls, email and internet usage.
