Position Summary

Wound and Enterostomal nurse is a registered nurse who works within the scope of nursing practice, provides treatment recommendation, monitors all complex wounds and provides staff education, utilizing the nursing process, assessment, diagnosis, planning, implementation and evaluation. Participates in Pressure injury prevalence and incidence study. Provides bedside care to patients with specialized needs with gastrointestinal and genitourinary and integumentary system disorders, abdominal stoma, wounds, fistulas, drains, pressure injuries, and continence disorders. Skills entail stoma siting, immediate post op assessment, treatment and patient care giver education and to promote patient advocacy. Contributes to positive outcomes and meets the cultural expectations of the organization by exhibiting professional behavior and clinical expertise. Must contribute to the achievement of the goals of the organization for patient, employee, and customer satisfaction. In addition to professional/technical duties, performs coaching and mentoring, collaborates with and motivates staff.

Standard Work Duties: Enterostomal Resource Nurse

  • Establish baseline for re- evaluation of plan of care by collecting data from EMR, patient/ family and inter-professional team (nursing, PT/OT/Dietitian/SW/ wound care specialist/ physician/ ARNP).
  • Review PMH, labs, nutritional assessment, Braden scale, support surface therapy. Determine wound etiology.
  • Recommends POC, evidenced based practice, cost efficient for the patient, staff and hospital.
  • Conduct a patient follow up and wound healing monitoring, provide documentation, re-assessment on effectives of POC weekly or biweekly, alter treatment plan until goal is met, patient is discharged or service is transferred to another provider.
  • Works rotating split weekends.
  • Uphold skills relevant to the ET/Wound care department. Stays current with educational evidence-based practice and updates, attends meetings, net learning opportunities to support a strong work group.
  • Acts as resource in areas of wound care to nursing staff.
  • Coordinates without patient program director/ Nurse manager and team members and discusses any issues that impact patient care received on weekends, holidays, and plan for patient discharge and follow up.
  • Ostomy care, education of patient, family and staff. Stoma siting/ marking.

Competencies & Skills

Essential:

  • One or more years of wound care nursing experience.
Nonessential:
  • Current Wound/Ostomy nurse.
Qualifications & Experience

Essential:

  • Associate Degree
Nonessential:
  • Bachelor Degree

Essential:

  • Basic Life Support

Other information:
Licenses Essential: Registered Nurse license or compact license, BLS.
Experience Essential: Three to five years acute care experience.
Certifications Preferred: WCC or WOCN
