Position Summary
Summary:
Responsible to the Team Leader, Charge Nurse, Shift Supervisor or Department Manager. Assist with patients under the direction/delegation of the Registered Nurse or licensed nurse assigned by the RN, as needed, and must be able to perform satisfactorily the specific patient care skills as outlined by the facility and the department(s). Must effectively interact with patients, families and other health team members. Must report pertinent information to the RN or licensed nurse
assigned by RN, as needed, and respond to requests for information in a timely manner. Assist with tasks to support patient care.

  • Standard Work Duties: Constant Observation Attendant
  • Observes patients for changes in condition or behavior and promptly reports these changes to RN or licensed nurse assigned by RN, as needed. Follows up as appropriate with supervisor regarding reported complaints, problems, and concerns 100% of the time. Report to RN reddened areas, N/V, amount and color of emesis, diarrhea, or any abnormal BMs, constipation, poor appetite, any complaints of pain, any complaints patient and family has or questions regarding Plan of Care, no urination, appearance of urine if anything other than clear, change in alertness, or if IV hurts, leaks or pump beeps. Complete COA/ROA flow sheet with every patient.
  • Beginning of Every Shift: Receive report from RN Introduction to the patient, know patients diagnosis, any recent act of violence, any precautions. Call light and phone is within patient's reach, unless ordered otherwise by the RN, and patient knows how to use it, Tidy the room, bedside table, floor and keep path to bathroom free from clutter, gather all anticipated supplies that will be needed. Fill water pitcher, provide fresh cup and linen.
  • Provides constant observation for patients being held under the Florida Mental Health Act and other patients as assigned. This includes accompanying patient to all medical procedures. Always maintain a clear exit for safety. It is suggested to sit on the side of the bed closest to the door or near the door depending on your patient.
  • Daily: Under the discretion of the supervising nurse assist with bath, assist with oral care (including dentures), brush hair, back rub'offer and administer if requested, soak feet, assist and/or monitor shaving Ask if patient has toothpaste, soap, toothbrush, etc., (request from patient care staff if needed), assist in changing bed linens as needed.
  • The ROA will have no more than 4 patients at one time to observe. The ROA will not be required to do daily care.
  • Every Shift: Report to the RN the percentage of diet consumed. Assist patient with ambulation to a chair or the bathroom. THROUGH OUT Shift: Communicates effectively with team members; maintains patient comfort, confidentiality and privacy and adheres to HIPAA standards. Follows all protocols and policies to ensure patient safety. Documentation including care provided and patient's behavior are recorded on the Constant/Roaming Observation Sheet. Place COA/ROA flow sheet in chart when completed. It is part of the permanent Medical record. Make sure RN completes safe room check list with you and all patient belongings are removed from room by Security.
  • EVERY 2 Hours: Assist patient care staff with turning patients. Observes their skin (heels, back, ears, shoulder blades, elbows, buttocks) and back of head and reports changes to RN or nurse assigned by RN. If needed, prop pillows on back and place pillow between knees. Reports complaints of pain to the patient care staff/RN.
  • MEAL times: Offer wash cloth so patient can wash hands before meals. Set up food trays. Feed patient, if needed. Patients with swallowing difficulty may need specific direction from RN prior to feeding patient.
  • THROUGH OUT Shift: Inspect all aspects of patient's environment to ensure patient's safety, and report correct unsafe conditions or report to RN or nurse assigned to patient. Assist patient to bathroom at least every 2 hours. Keep door propped open at all times. Have a clear visual of patient and arm length distance if a fall risk. Offer assistance with toileting. Offer wash cloth so patient can wash hands after bathroom. Assist RN, LPN, and PCA with care as needed within COA/ROA job description parameters. Maintain clean environment. Place soiled linen in bag and place at door for removal
  • COMMUNICATION: Active listening to patient. No judgmental statements or opinions. Report any reports of abuse to supervising nurse. Report any signs or statements made by patient about harming themselves, or others to supervising nurse.

Other information:
Experience Preferred: Three to six months prior experience either in acute patient care, long-term care, home health care, or in a licensed child care facility preferred.
Certification Essential: BLS
