Position Summary

The Cancer Center Nurse Navigator position assists cancer patients through their entire course of breast cancer care. The Nurse Navigator is an experienced oncology nurse whose role is to coordinate medical care, facilitate communications amongst healthcare providers, assess and provide for the emotional and educational needs of cancer patients, and to streamline the entire plan of care for the cancer patient. Performs other related duties as assigned.

    • Standard Work Duties: Cancer Center Cancer Nurse Navigator
    • Facilitates an evidence-based, multidisciplinary, multispecialty approach throughout the care continuum. Remains current in evidence-based practice and new emerging trends. Participates in the development of practice standards including pre-op, post-op, and discharge instructions.
    • Develops, tracks, and benchmarks (in collaboration with the physician/administrative team) programs outcomes related to clinical/operational goals data analysis to align with overall goal of tracking to support efforts related to pay for performance reimbursement.
    • Promotes to cancer nurse navigation program awareness within the physician community and the community at-large. Participates in marketing through physician visits, dinners, educational seminars, and community lectures. Develops appropriate relationships to maximize the cancer nurse navigation program effectiveness and facilitates program problem resolution across departmental, campus, and physician/provider lines. Actively participates in team development, achieving dashboards, and in accomplishing department goals and objectives.
    • Ensures the provision of high quality cancer care by guiding patients through their diagnosis, treatment, and recovery process. Collaborates with the Center Program Leaders to meet the strategic goals and initiatives of the program. Facilitates communication amongst healthcare providers and establishes a collaborative relationship with these providers to ensure a smooth coordination of care for breast cancer patients.
    • Collaborates with the Center Program Leaders, patients, and multidisciplinary healthcare team members to develop, update, and coordinate a patient's pre and post op plan of care to streamline and accelerate care. Provide emotional and psychological support of breast cancer patients and their caregivers. Identifies the educational needs of cancer patients and provides materials and/or programs to meet these needs. Present formal education programs to specific audiences on breast cancer.
    • Maintains updated cancer resource and educational materials. Assist in the development, collection, and evaluation of performance improvement measures for NAPBC accreditation process.
    • Acts as a liaison between LRHHCC and other ancillary services to ensure specific needs and concerns of the cancer patient are addressed. Participate in LRHHCC community outreach educational and screening programs.
    • Participates in the development and implementation of goals and strategic initiatives for LRHHCC's cancer programs.
    • Monitors patient progress through diagnosis, treatment, and recovery to ensure optimal outcomes are achieved.
    • Demonstrates exceptional customer service skills to exceed cancer patient expectations with LRHHCC's programs.

  • Associate Degree

Experience Essential:
  • Minimum two years of nursing experience.
Experience Preferred:
  • Physician outpatient practice setting experience preferred. Experience in case management desirable.
Certification Essential:
  • OCN Certification or Nurse Navigation Certification
  • Per department or hospital requirements and relevant policies.
Licenses Essential:
  • Florida Registered Nurse license.
