    JOB SUMMARY
    The Transition Specialist provides educational, classroom and community/worksite assistance for students with special needs as they transition from a K-12 educational setting into the adult world of work, vocational school and/or higher education. The position works with other agencies, performs instructional activities, promotes student safety and well-being, and reinforces classroom programs, independent living strategies and worksite requirements. The position has specific responsibility for supervising and instructing special needs students both in district facilities and at external community locations and observing and documenting student progress.
    ESSENTIAL DUTIES AND RESPONSIBILITIES
    Essential duties of this position include the following tasks. Employees in this position perform some or all of the following tasks. Other duties may be assigned at any time.
    Collaboration-Internal
    • Works with learning specialists, resource room teachers, district psychologists and other staff on identifying and planning for IEP transition goals and objectives.
    • Demonstrates knowledge and understanding of all district policies and procedures.
    Collaboration-External
    • Participates with the planning committee for CCC transition groups, JDN, OETA, C-Tec, Occupational Skills Training and other vocational training programs.
    • Participates as a community partner with Vocational Rehabilitation.
    • Attends relevant trainings offered by the Department of Human Services, as allowed.
    • Works with neighboring school districts and their post-high school transition programs.
    • Contacts businesses regarding job opportunities and writes and circulates job hot sheets for special services.
    • Works with agencies including Vocational Rehabilitation, Clackamas County and Multnomah County developmental disability agencies, CCC, PCC and other outside placement providers.
    Student Evaluation and Assessment
    • Assists teachers and other specialists with testing and assessment of individual student skills and abilities for potential job placement or other potential work experience.
    • Coordinates referrals for CESD pre-vocational assessments.
    Training/Instruction
    • Directs group and individual trainings in driver’s education, locating jobs or work experience opportunities, resume development and interview techniques and obtaining a food handler card or other necessary work-related documents.
    • Assists students with public transportation route/trip planning, waiting, boarding, riding, exiting, and other public transportation essentials. Helps students obtain applicable monthly passes or other passes/tickets/ fare-prepayments.
    • Assists vision-impaired students with strategies and procedures for safely crossing streets, rail, light rail and streetcar tracks and similar thoroughfares.
    • With resource room teachers, initiates enterprise projects for students.
    Work Experience
    • Arranges Career Related Learning Experiences (CRLE), develops job shadows and tours of employer facilities and arranges for an employer introductions and interviews.
    • Provides job-training assistance to students obtaining work experience in the district’s facilities and through external workplace or job site placements.
    • Develops job training programs and training manuals for vocational experiences.
    Education
    Assists students with trips to CCC and PCC to explore potential classes and programs.
    General
    • Updates information regarding new programs, agencies and training opportunities for students. Shares this information with other specialists, teachers and case managers.
    • Compiles transition resources list for district-wide distribution.
    • Maintains accurate and up-to-date case files on all assigned students. Completes necessary data records and reports.
    • Attends workshops and staff development classes to remain current on behavioral and performance assessments, data collection procedures and transition strategies.
    • Maintain current knowledge of job market and vocational training opportunities.
    • Consistent, daily and punctual attendance.
    NON-ESSENTIAL DUTIES AND RESPONSIBILITIES
    Non-essential duties of this position include the following tasks. Employees in this position perform some or all of these tasks. Other duties may be assigned at any time.
    • Operates various office equipment including telephones and photocopy machines.
    • Attends in-service trainings, staff meetings and other meetings as assigned.
    • Reports issues to the principal and the appropriate authorities including safety, sanitary and fire hazards, vandalism, thefts and errant wildlife.
    OTHER REQUIREMENTS
    • Consistently follows all applicable federal and state laws and all district policies, rules and supervisor directions.
    • Observes ergonomics of sedentary office work including placement of computers and monitors, and sitting alignment.
    • Observes proper lifting techniques and other strategies to avoid accidents and personal injury while performing job duties.
    • Performs other duties as assigned.
    SUPERVISORY RESPONSIBILITIES
    All school employees have some responsibility for supervising students and assisting in maintaining a safe environment.
    QUALIFICATIONS
    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.     • Education and Experience: A Bachelor’s Degree in Education or related field is is required. Three (3) years of previous experience dealing with individuals with disabilities is required. Demonstrated working knowledge of disabling conditions is required.
    • Interpersonak Skills: Works well with others from diverse backgrounds. Focuses on solving conflict, maintaining confidentiality, listening to others, keeping emotions under control, remaining open to others’ ideas and contributing to building a positive team spirit. Demonstrates the ability to remain calm and professional in an environment with frequent interruptions. Interacts with staff and students appropriately.
    • Language Skills: Ability to communicate English fluently both verbally and in writing, including proficiency in spelling and grammar. Ability to respond to common inquiries or complaints from students, parents, staff, regulatory agencies, vendors and members of the community. Ability to read and interpret documents including governmental regulations, safety rules and operating and maintenance instructions/ instruction manuals. Ability to prepare routine reports and correspondence with high degrees of accuracy and professionalism.
    • Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to calculate rates, ratios, percentages and proportions. Ability to draw and interpret bar graphs.
    • Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in oral, schedule or diagram form. Ability to deal with problems involving more than one variable.
    • Computer Skills: Ability to effectively use office computers and internet access for email, research, word processing, database manipulation, visual presentations and general web-based applications.
    • Other Skills and Abilities: Ability to exercise good judgment and work effectively in a dynamic environment.
    • Certificates, Licenses, Registrations: A valid First Aid card and Oregon Driver’s License are required. Must have or be able to obtain various certificates, licenses and/or registrations as required by federal, state or local law(s) and/or determined by the district.

    The work environment is a regular and/or special needs classroom setting and other activity/learning areas. The noise level of the work environment is usually low to moderate, but occasionally high depending upon the location, the student population and current activities. The employee occasionally will be required to supervise students during their arrival/departure and recess. The employee is occasionally exposed to wet or humid conditions and outdoor weather conditions, including seasonal heat and cold. The employee may occasionally be exposed to contagious diseases, bodily fluids and blood-borne pathogens.
    The employee may be exposed to moving parts of nearby machinery, a variety of cleaning agents, some hazardous chemicals and fumes, risk of electrical shock and mechanical vibration.
    Special Requirements:
    The person selected for this position must be able to provide: proof of educational requirements, verify identity and employment authorization, complete a pre-employment record check and be approved by the State Department of Education after having a Fingerprint-Based Criminal History Verification completed. The selected candidate will pay $10-$20 for a set of fingerprints and $66.00 for the verification check.
