Social Service Specialist shall be responsible for the providing trauma focused therapeutic services and case management in accordance with the resident's individualized service plan and consultation with identified on-site and off-site support systems. Social Service Specialist is administratively responsible to the Director of Social Services.
1. Conduct and assist with intake and discharge procedures such as intake assessments mental health screenings and discharge summaries.
2. Implement TBRI based interventions
3. Document weekly social skills and independent living skills group and individual training sessions.
4. Perform and document social services casework in resident's case records.
5. Facilitate and promote family engagement between resident and identified support system
6. Participate in monthly multi-disciplinary team staffing for all residents.
7. Coordinate with resident and guardian for necessary services provided by outside agencies such as mental health and substance abuse treatment. This may include exploring resources, scheduling appointments and transporting residents to appointments. Social service specialist engage resident in selecting their outside therapist by providing the resident a voice in choosing from a list of identified providers.
8. To perform any services the Director of Social Services may request as it pertains to the care of residents or proper administration of the social services programs as required by facility or state licensing policies.
9. To serve as on-call administration supervisor on a rotating basis with the Director of Social Services, Unit Supervisors, Evening Coordinator, Program Director and Y.E.S. Employees (with a Bachelors degree)
10. Gather information from residents and legal guardians to prepare social histories of residents and families as needed for evaluations ordered by the courts, intake assessment, documentation for medical services, service plans, etc.
11. Train and assist Y.E.S. Employees in working with residents on goals and issues identified in the resident's service plan, as well as placement stabilization and crisis interventions.
12. Train and assist Y.E.S. Employees with conducting Nurture Groups.
13. Coordinate the discharge/aftercare process which includes: the transitional/multi-disciplinary team meeting, family sessions to prepare for reunification or a less restrictive environment, coordination of care/scheduling follow up appointments, and six months of aftercare follow-up;
14. Assist the family to develop constructive and personally meaningful ways to support the resident's experience in the facility, through counseling and Education on TBRI’s Connecting Principles, and referral to specific services, as appropriate;
15. Coordinate with DCFS to ensure engagement regarding resident and the child of a resident.
Qualifications:
Social Service Specialist shall have a bachelor's degree from an accredited college or University and at least two years of direct practice in social work, mental health or a related field.
