The Telemonitoring Registered Nurse serves an integral role in our CareLink Program.

• Maintain an up to date knowledge base of the newest practices and equipment in the home telemonitoring marketplace including reimbursement issues
• Assist with staff education on all elements of home telemonitoring, including but not limited to knowledge of telemonitoring definitions and concepts, equipment set-up and maintenance, cleaning and infection control, selection of the appropriate patient population and agency specific protocols and policies under the direction of the TM Coordinator or agency DON
• Perform the role of liaison between the home health agency and the physician
• Perform the role of liaison between the patient and the home health agency staff based on results of data collection and interventions ordered by the physician
• Perform installations of telemonitoring equipment and instruct patient/caregiver on use of the equipment
• Perform de-installations of telemonitoring equipment as directed by TM Coordinator or Agency DON
• Notify the physician of parameters out of range and coordinate follow through with orders obtained
• Understand and oversee all aspects of the home telemonitoring equipment functioning, including but not limited to basic equipment functioning and data transmission in the patient home and in the home health agency, management and return of malfunctioning equipment, equipment maintenance and cleaning including infection control, equipment storage
• Oversee and monitor data collection results on all telemonitored patients daily at multiple sites, including but not limited to ensuring proper documentation is complete and distributed to each agency on all patients referred for monitoring, daily management of monitored patients with alerts, documentation of all patient interactions in vendor software, communication with agency staff on patient alerts/problems, distribution of VS trends to be placed in the patient's medical record
• Other tasks as assigned

• 1 year clinical experience
• Home Health and Cardiac Specialty experience preferred
• Current nursing licensure
• Knowledgeable of home care accreditation and compliance guidelines
• Computer proficiency (limited Microsoft excel)
