Description:

Summary: A registered nurse that assumes the responsibility and accountability for the coordination and facilitation of services needed to care for a patient during an entire episode of care. To provide ongoing support and expertise though comprehensive assessment, planning, implementation and overall evaluation of individual patient care needs. Responsibilities include: Oversees Case Management/Social Services/Discharge Planning/Project Red/Transitional Care Management/Utilization Review Programs and Staff; Serves as patient advocate; Investigates and reports adverse occurrences, performs staff education related to oversights areas; Facilitates interdisciplinary Rounds and/or conferences to review treatment goals, optimize resource utilization, provide patient/family educations and identify post-discharge needs; Communicates with physicians at regular intervals; Supervises and audits CM/SS documentation; Directs and participates in the development/implementation of evidenced-based practice policies/procedures; Directs and participates in the development/implementation of evidenced-based practice polices/procedures for patients with special cases or patient needs; Attends assigned meetings or projects timely and participates in a positive, collaborative, team approach.

The overall goal of the position is to:

  • Enhance the quality of patient management while also enhancing the involvement of patients/family during their hospitalization to increase patient satisfaction
  • Enhance Discharge Planning and Transitional Care Management to promote safe, seamless transitions back into the community
  • To promote continuity of care and patient education for management to improve health outcomes
  • To promote cost effective care with utilization review, finance and HIM
  • Coordinate the Case Management/Discharge Planning/Transitional Care Management functions in patient care, discharge, and home planning process through an internal multi-disciplinary process, external service organizations, agencies and healthcare facilities.
  • Promote an effective work culture while also motivating team members

Education Requirements: Degree in Nursing from an accredited college or university, Bachelor’s degree required or obtain within 2 years of hire. Current Kansas Registered Nursing license required. Minimum of three (3) years of acute care experience and sufficient knowledge and experience in understanding of treatment required. Minimum of 3 years of Case Management and Utilization Review experience preferred. Previous leadership experience preferred. Computer literacy is required and recent word processing, spreadsheet competency is preferred.
