Social Worker Qualifications:

• Master's degree in social work from an accredited school of social work
• Master's degree in psychology, gerontology or counseling from an accredited school and has >1 years of experience providing social services in adult day care or related field
• Social Worker assistant: BS degree in social work (BSW), psychology or sociology

Social Worker Responsibility:

• Interviewing and screening all applicants/referrals
• Attending and participating in all the multidisciplinary team (MDT) meetings.
• Developing the plan of care in collaboration with the participant's identified goals and/or concerns.
• Completing, at a minimum, quarterly and six-month reassessment which includes evaluating and reviewing the progress made towards the participant's identified goals.'
