We seek a passionate, professional, experienced Bilingual Director of Intake for Home Care who oversees the intake process and ensures clients’ seamless transition into home care services. This role involves coordinating with referral sources, assessing client needs, and managing the intake team to deliver high-quality care, as well as overseeing the operations of our organization. You will be responsible for supervising a team of professionals working and providing administrative support.

Duties and responsibilities:

• Oversee the intake process for new clients, collaborating with healthcare professionals, physicians, and referral sources.
• Conduct thorough assessments of clients’ needs, medical histories, and home environments.
• Collaborate with healthcare providers and other professionals to determine the appropriate level of care and services.
• Ensure timely and accurate completion of intake assessments and documentation.
• Develop and maintain positive relationships with referral sources, including hospitals, physicians, rehabilitation facilities, and community organizations.
• Communicate effectively with clients, families, and healthcare professionals regarding the intake process and home care services.
• Work closely with other departments, such as nursing, scheduling, and billing, to ensure smooth client transitions.
• Supervise and lead the intake team, providing guidance and support.
• Generate reports on intake activities, outcomes, and key performance indicators.
• Conduct training sessions for intake staff to ensure compliance with policies and procedures.
• Ensure all intake documentation meets regulatory requirements and agency standards.
• Conduct regular reviews to identify areas for improvement and implement changes accordingly.

Requirements:

• High School Diploma or equivalent required.
• Preferred one-year experience as an Intake Director in-home care or:
• Previous experience in a leadership role in home care, healthcare administration, or a related field.
• Knowledge of home care regulations, assessment processes, and care planning
• Understanding of the Insurance verification process
• Proficient in Microsoft Office (Outlook, Word, Excel).
• Must be organized and detail-oriented with excellent listening, written, and oral communication skills.
• Strong leadership, organizational, and interpersonal skills.
• Self-motivated, able to work independently and with teams, multitask, and make sound decisions when needed.
• Proven experience in program development and management
• Ability to effectively supervise and lead a team
• Bilingual: Spanish/English (other languages is a plus)

Job Type: Full-time

Pay: From $85,000.00 per year
