La Causa, Inc is Hiring a Care Coordinator - CSC/CCS Youth Near Milwaukee, WI

PRIMARY FUNCTION: As a member of a multidisciplinary team, the care coordinator is responsible for providing care coordination to youth and young adult with high-level needs who are dually diagnosed with mental health and developmental disabilities toward helping them and their families to develop an individualized plan that can be used to help obtain their vision toward being happy, health, and safe.
PRIMARY DUTIES:

1. Facilitate all aspects of services for enrolled youth (transport participants as needed, assemble teams, assess needs and strengths, develop an individualized care Plan of Care (POC), crisis plan, and/or other care plan, complete functional screens, provide advocacy for the youth and family, provide or secure additional services, etc.). E
2. Coordinate care for the youth and family utilizing mental health (CCS) and disability (CLTS) services in collaboration with the other CSC service providers toward assuring that youth are received comprehensive interventions toward meeting their identified goals. E
3. In collaboration with the CSC team, monitor and negotiate provision of services and agreements, modify the POC, and authorizations when new services are added and/or existing services discontinued, etc. E
4. Follow and ensure all legal, organizational and contractual requirements, laws and policies. E
5. Prepare, complete, distribute, and/or submit required items by due dates. E
6. Attend meetings and professional development activities, as requested. E
7. Establish and maintain good communication, collaboration, and cooperation with all stakeholders. E
8. Follow all duties of a mandated reporter. E
9. Perform other related duties, as assigned. E
10. Be a team player. E
Requirements
PHYSICAL DEMANDS:

The employee is regularly required to stand, sit, reach, stoop, bend, and walk. Infrequent lifting may be encountered.
WORKING CONDITIONS:
Work is generally performed in field and office environments, including travel. Local travel required; some state-wide travel as needed. Flexible hours required; as dictated by program needs.
REPORTS TO: Clinician/Supervisor - CSC/CCS-Youth
MINIMUM QUALIFICATIONS REQUIRED:
1. Bachelor's Degree from an accredited school in Social Work or related field (i.e.; psychology, sociology, counseling). Certified Social Worker certificate (Preferred) ; Master's Degree in Social Work or related field (i.e.; psychology, sociology, counseling) (Highly preferred) .
2. One (1) year of continued experience providing mental health services.
3. Two (2) years of continued experience serving youth and young adults with mental health and developmental disabilities (Preferred)
4. Bilingual (Spanish and English), spoken and written. (Desirable)
5. Neat orderly person with great organizational skills.
6. Must be a people person with a great disposition and outlook on life, who enjoys working with others, is upbeat and pleasant to work with and is a good listener.
7. Knowledge and experience with Microsoft Office suite.
8. Must be willing and able to work flexible hours as needed.
9. Excellent communication and documentation skills, written and verbal
10. Must pass all background checks.
11. Transportation, valid Wisconsin Driver's license, state minimum auto insurance and must meet La Causa, Inc. driving standards.
