KVC Kansas Health System is Hiring an Education Transition Specialist Near Hiawatha, KS

KVC is seeking an Education and Transition Specialist!!! The Education and Transition Specialist provides transitional independent living resources to Case Managers for youth that are 17 and over in the foster care contract.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Other duties may be assigned according to qualification of staff and development of programs.

  • To continue to build a child welfare system that is safe, secure, and stable; family centered, connected to the community; and which relies on evidence-based practice necessary to support contract services in Kansas.
  • Schedule and host transition meetings for youth aging out of care.
  • Compile and review all available educational information at the time of referral.
  • Attend case planning conferences for those youth that are 17 years of age and older to explain DCF benefits.
  • Complete the Education Enrollment Information for School Placement Form (EEISPF) and send to enrolling schools
  • Assemble transition packets to disseminate to Case Managers and youth that outline benefit package available through DCF.
  • Locate and maintain three scholarship opportunities for youth's qualification for ETV benefits.
  • Request Individualized Education Plans (IEP) for children in care.
  • Recruit and support youth at the regional council level.
  • Act as a liaison between the school district and the case management team to assist in obtaining information.
  • Provide Job Corp contact information for applications, interviews, and tours.
  • Provide information for Case Managers to assist youth in applying for post-secondary education including completing financial aid packets, ACT/SAT fee waivers, tuition waiver and post-secondary education applications, scheduling tours and examining housing options.
  • At transition meetings, assist youth in completing extended medical card application.
  • Notifies current school when a child may be leaving the district and ensure the youth recovers all completed school credits.
  • Participates in the Best Interest Determination meeting with the case management team to advocate for children to remain in the same school district whenever possible.
  • Consult with DCF Coordinators for youth to access benefit package when leaving care.
  • Review transition forms and medical card extension forms before submitting to DCF. Attend designated permanency team staffings/meetings.
  • Train team members on the requirements of the Every Student Succeeds Act.
  • Complete and submit required reports including monthly statistics.
  • Complete required number of in-service training hours annually.
  • Support continuing education by collaborating with the case management team to obtain homework/study materials and/or access alternative options when a child is unable to attend school due to placement disruption, suspension or expulsion.

Education:

  • High School Diploma

Licensure/Certification:

  • Valid driver's license and auto insurance

Experience:

  • Requires Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as Outlook email.

Preferred Experience/Skills:

  • Previous office experience and experience with high-risk children or foster care children is preferred.
