KVC Health Systems is Hiring a Family Navigator Near Topeka, KS

KVC Kansas represents one of the strongest and broadest child welfare and behavioral healthcare continuums in the nation. We are responsible for the care of all children who are served by the Kansas Department for Children and Families in the Kansas City Metro and East Regions. KVC’s clientele represent diverse backgrounds and KVC is seeking to enhance and expand our diversity in order to better serve our clients.

KVC is seeking a Family Navigator!!! The Family Navigator will assess the family in order to connect them with services to help meet their needs and engage them in the TANF 2Generation Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Other duties may be assigned according to qualification of staff and development of programs.

  • Conduct Initial Family Assessments:
    • Meet with families to establish rapport and understand their unique circumstances.
    • Utilize standardized tools and interviews to assess the family's strengths, needs, and challenges.
    • Document assessment findings accurately and comprehensively.
  • Identify and Prioritize Needs:
    • Analyze assessment data to identify key areas requiring intervention and support.
    • Prioritize needs based on urgency, severity, and impact on family stability and well-being.
  • Develop Individualized Service Plans:
    • Collaborate with the family to develop personalized service plans tailored to address identified needs.
    • Utilize a strengths-based approach to leverage existing resources and capacities within the family and community.
    • Ensure service plans align with program goals and objectives.
  • Connect Families to Services:
    • Refer families to appropriate services within KVC programs/groups or community-based organizations.
    • Coordinate referrals and facilitate access to resources, including but not limited to housing, healthcare, education, employment, and financial assistance.
    • Advocate for families to ensure they receive timely and culturally responsive services.

Education:

High School Diploma

Licensure/Certification:

Bachelor’s degree in social services field

Current Driver’s License and Auto Insurance

Experience:

Requires Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as Outlook email

Preferred Skills:

Experience working with children and families in need
