KVC Health Systems is Hiring a Case manager Near Olathe, KS

KVC is seeking an Independent Living Case Manager!!!

The Independent Living Case Manager provides case management services to children and families in order to facilitate safety, permanency, and well-being.

Continue to build a child welfare system that is safe, secure, and stable; family centered, connected to the community; and which relies on evidence-based practice necessary to support contract services in Kansas.

The primary role of this Case Manager will be to work with young adults who are individuals with developmental delays to transition then into adulthood.

Office location is negotiable. Extensive driving required. Fleet vehicles are available. In the event a fleet vehicle is not available, use of a personal automobile with approved mileage reimbursement is allowed.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned according to qualification of staff and development of programs.

  • Provide in-home services to clients and their families.
  • Complete all worker / child and worker / parent contacts as directed by supervisor
  • Supervise parent / child and sibling visits as required
  • Transport clients to visits and appointments as needed
  • Gather information on children and families functioning and integrate that information into comprehensive assessments
  • Provide on-call crisis intervention
  • Provide Independent Living services to children as needed
  • Assist parents in finding community resources such as housing, utilities, and mental and physical healthcare
  • Provide referrals for mental health services and any other required service for the children and / or family
  • Develop safety plans and aftercare plans with families
  • Organize and facilitate case planning conferences
  • Complete court reports
  • Assess viability of relative and kinship placements through home studies
  • Testify in termination of parental rights trials and other hearings
  • Complete child social histories
  • Provide reintegration and adoption services
  • Maintain working relationships / communications with DCF, the court, and other community partners ·Document all contact with clients and collaterals;

complete case activity logs and all related documentation; maintain client files

  • Maintain communication with foster homes
  • Attend school meetings as needed
  • Complete all court orders as directed by the court
  • Complete adoption home studies and attend all required trainings

Minimum Requirements :

Education : A bachelor’s or master’s degree, in a field that is able to be licensed by the BSRB, from an accredited college or university.

Licensure / Certification : Licensed by the Kansas BSRB.

Experience : Previous experience in children and family services preferred
