Responsibilities:

• Demonstrates excellent phone skills including inquiry calls, explanation of assessment processes

• Documents all inquiry calls, completely, to include all applicable information on prospect form in Wellsky.
• Gather information about background and current situation of patient.
• Interview patient to assess all aspects of situation/problem.
• Make referral or arrangements for treatment or support services.
• Conduct intake assessments.
• Prepare referral documents.
• Prepare intake reports.
• Ensures proper documentation in all position activities, following federal and state guidelines.
• Strives to always do the right thing for the patient, coworkers
• Adheres to established standards, policies, procedures, protocols, and laws.
• Supports and demonstrates fiscal responsibility through supply usage, ordering of supplies, and conservation of facility resources.
• Completes required trainings within defined time periods, as established by job description, policies, or hospital leadership
• Exemplifies professionalism through good attendance and positive attitude, at all times.
• Maintains confidentiality of patient and staff information, following HIPAA and other privacy laws.
• Effective uses Microsoft products such as Excel, Word, and Outlook to completely daily reports

Pay: $20.00 - $25.00 per hour
