Position Overview:
We are seeking a compassionate and dedicated Licensed Social Worker (LSW) to join our team at Kulana Malama. The ideal candidate will play a crucial role in providing support to medically fragile children and their families, contributing to the overall mission of improving their quality of life and providing much-needed respite care.
Responsibilities:
Family Support:
- Work closely with families to assess and address psychosocial needs, providing emotional support and counseling.
- Collaborate with healthcare professionals to develop and implement comprehensive care plans tailored to the unique needs of each child.
Advocacy and Empowerment:
- Advocate for the rights and preferences of medically fragile children, ensuring their voices are heard and respected.
- Empower families by providing information on available resources and facilitating connections to support networks.
Collaboration and Coordination:
- Collaborate with interdisciplinary teams to ensure a holistic approach to patient care.
- Coordinate with external agencies and community resources to enhance the overall support network for families.
Respite Care Enhancement:
- Contribute to the development and implementation of respite care programs, recognizing the importance of providing families with well-deserved breaks.
