Base Salary: $345,000 plus $3600 CME expenses (paid out in equal amounts per pay check minus applicable taxes)
3% annual increase in salary
One-time Sign-on bonus $14,250 payable ninety (90) days after Employees Start Date
Retention bonus: Annually at the end of each year of employment based on Employees Start Date for years two (2) through five (5).End of Year 2: $7,500
End of Year 3: $10,000
End of Year 4: $12,500
End of Year 5: $15,000
Annual Employee Quality Bonus: $4,000 per year should Employees quality notes be rated as Acceptable or better for the calendar year
Additional compensation including:$125 per hour for additional medical director hours over 20/month
$75 per patient if more than 15 patients are seen in a day (20 patients if on the weekend)
$30 per patient if provider works a vacation
$150 per consult for any ED/CL Consults at Indian River Hospital if the attending or patient doesnt want tele
$150 for second opinion consultations
$1500 per day for weekend coverage, optional
$350 per night for telephonic call, optional
$40/hour plus $150/encounter for on-demand tele ed/cl coverage, optional

PTO Days: 15 per year accrued bi-weekly
Floating Holidays: 5 per calendar year to be taken on a day of Employees choosing throughout the year
CME Time: 3 days per calendar year for completion of CME work
