Home Care Nurse (RN or LPN) Job Description:

• Conduct the admission visits. The nurse will get the opportunity to meet with new enrolled clients and family members to discuss their needs, review admission paperwork, and to develop the client's care plan.
• Conduct ongoing caregiver supervisory visits. The visits are to ensure that our clients are satisfied with service delivery, and to ensure caregivers are following the care plan as outlined.
• Conduct quality assurance visits. These visits are to ensure that our caregivers are providing competent in-home care services to our clients, following dress code, and to evaluate their job performance.

Qualifications:

• Current Registered Nurse or Licensed Practical Nursing license with the state of South Carolina
• Must receive a negative TB test, negative Drug Test and pass a criminal background check
• 2-years of home care, home health, public health or long term care nursing experience.
• Be willing to travel to client’s homes, and conduct home visits.
• Must possess a valid driver's license, with proof of valid auto insurance
• Must demonstrate excellent oral and written communication skills and the ability to listen effectively
• Must have the ability to work independently, maintain the confidentiality of the information and meet deadlines
• The ability to demonstrate good interpersonal skills and good decision making.

Work Attire
