On-call After Hours LPN or RN Home Health this crucial role will help ensure the well-being of our home health patients after hours.

Primary shifts needed: Monday - Thursday evenings 5pm - 8:30 am.

Whether you are an experienced home health nurse or new to the field, we offer incredible mentors to support your growth. What we need most are solid nursing skills and an open and willing heart to learn.

Responsibilities:

• Respond to after hours calls from Home Health patients/caregivers (Call volume varies from 0-5 calls/shift).
• Conduct phone triage and assess patient needs, determining the necessity of after-hours visits.
• Provide skilled nursing care, including catheter changes and other essential tasks, as needed.
• Document patient interactions and care provided in a timely and accurate manner and send summary at end of shift.
• Maintain clear and effective communication with patients, families, and other healthcare professionals.
• Collaborate with the on-call administrator when necessary for complex situations.
