Overview:
We are seeking a detail-oriented and compassionate individual to join our team as an Intake Coordinator. As an Intake Coordinator, you will play a crucial role in providing administrative support and coordinating patient intake processes. This position requires strong organizational skills, knowledge of medical terminology, and the ability to work effectively in a fast-paced environment.
Responsibilities:
- Greet and assist patients , families and facility case managers over the phone, providing exceptional customer service
- Collect and verify patient information, ensuring accuracy and completeness
- Schedule appointments and coordinate with medical staff to ensure efficient patient flow
- Obtain necessary authorizations and referrals for the patient
- Maintain confidentiality and adhere to HIPAA regulations when handling patient information
- Enter patient data into electronic medical record systems (such as HCHB) accurately and efficiently
- Assist with medical administrative tasks, including filing, faxing, and scanning documents
- Collaborate with other healthcare professionals to ensure seamless coordination of care
-Have knowledge of cloud based hospital referral systems such as Careport
Qualifications:
- Previous experience in Hospice intake setting is required
- Strong customer service skills with the ability to empathize with patients
- Knowledge of EMR systems and terminology
- Proficient in using electronic medical record systems (such as HCHB) or similar software
- Excellent organizational skills with the ability to multitask and prioritize workload
- Attention to detail and accuracy when entering patient information
- Familiarity with office equipment (computers, printers, scanners)
- Ability to maintain confidentiality and adhere to HIPAA regulations
