Position Overview:

We are seeking an experienced and compassionate Clinical Manager (RN/LPN) to lead our dedicated team in providing high-quality care to individuals with memory-related challenges within our assisted living community. As a Clinical Manager (RN/LPN), you will play a pivotal role in ensuring the physical, emotional, and cognitive well-being of our residents while fostering a warm and engaging environment.

Responsibilities:

  • Develop and implement person-centered care plans tailored to individual residents' needs, ensuring their safety and comfort.
  • Lead and mentor a team of caregivers, providing guidance, training, and support to promote professional growth and job satisfaction.
  • Collaborate with interdisciplinary teams to coordinate resident care, ensuring seamless communication and collaboration.
  • Conduct regular assessments and evaluations to monitor residents' progress, adjusting care plans accordingly.
  • Maintain accurate and up-to-date documentation of residents' medical records and care plans.
  • Foster a positive and inclusive atmosphere by promoting social engagement and meaningful activities for residents.
  • Stay updated on the latest research, trends, and best practices in memory care to enhance the quality of services provided.

Qualifications:

  • Current Santa Fe nursing license (RN/LPN) and CPR certification.
  • Proven experience in memory care or geriatric nursing, preferably in an assisted living or memory care facility.
  • Strong leadership skills with the ability to motivate and inspire a diverse team.
  • Excellent communication and interpersonal skills to effectively interact with residents, families, and staff members.
  • Demonstrated knowledge of dementia and memory-related disorders.
  • Ability to multitask, prioritize responsibilities, and make sound decisions in a fast-paced environment.
  • Compassionate, patient, and empathetic approach to resident care.
