Case Manager I (838)
Tulare, CA

Under minimal supervision, provides all types of Case Management services to SMI/SED clients. This position is an integral part of a treatment team. The Case Manager I will develop and assist in the implementation of life skills, training of individuals/ clients, wraparound services, and medication management.

How will I contribute

• As part of a service delivery team, provide services to mental health, substance abuse and consumers with co-occurring disorders
• Maintain clear, accurate and concise consumer records, in a timely manner.
• Assist with program design and implementation.
• Provide professional linkage/consultation with peers, professionals of other disciplines and outside entities.
• Enter all service information in the Tulare County HHSA Management Information System (AVATAR) by completing Electronic Service Activity Logs (ESALs) at the end of every shift.
• Perform other duties as assigned.

What I bring

• High School Diploma or equivalent plus two years of related experience, or B.A. degree with no experience
• Class C Driving Requirements
• Avatar processing, One Point for timesheet, assisting clinician with assessments, interagency and intra agency consultation, knowledge of community resources.
• Ability to relate to others comfortably and be able to establish and maintain effective relationships.
• Ability to communicate effectively in writing.
• Good organizational skills and communicates well with others.
