Summary

  • JOB SUMMARY
      The Enterstomal Therapy nurse provides a comprehensive ostomy, skin, and wound care program for inpatients at KDMC. The ET nurses act as a consultant to access, plan, implement, evaluate, and reassess the care of patients with abdominal stomas, acute/chronic wounds, and incontinence. The ET nurse provides services to patients ranging from adult through the geriatric developmental life span.
  • DUTIES AND ESSENTIAL FUNCTIONS
    • Effectively assesses and reassesses patients based on criteria that are both age and culturally appropriate for the patient; Assesses/reassesses patient needs using objective and subjective assessment of the physical, psychological, emotional, spiritual, cultural, and environmental needs of the patient.
    • Overall performance rating of below indicated essential job duties:
    • Identifies appropriate patient needs and nursing diagnoses from the collected assessment data and plans care appropriately; Formulates and documents a plan of care based on assessment and in collaboration with the patient, family, and/or significant other, as well as appropriate members of the multidisciplinary patient care team.
    • Updates and documents the plan of care daily and as indicated based on ongoing assessment; Administers medication correctly/safely utilizing five rights, two patient identifiers and double checks of high risk medications per policy.
    • Performs nursing procedures, interventions and actions safely according to hospital policy/procedures; Consistently and thoroughly completes documentation on correct forms per medical center/unit policies.
    • Coordinates and provides discharge planning and patient and family education throughout hospitalization; Provides care that is age appropriate and culturally sensitive; Evaluates the appropriateness of individual patient’s response to the nursing care and modifies the plan as evidenced by chart documentation.
    • Recognizes change in patient’s physical and mental status and informs physician and/or other health care professionals as evidenced by chart documentation.
    • Participates in Performance Improvement initiatives by evaluating outcomes of patient care and participating in data collection.
  • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
      Minimum requirement:
      • Graduate of a school of nursing and maintains current licensure as a Registered Nurse in the Commonwealth of Kentucky and other state(s) as required.
      • Achieves and maintains current American Heart Association Basic Life Support (BLS) Healthcare Provider Certification.

    • Constantly required to communicate with patients and other care team requiring talking and hearing.
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
    • Occasionally required to position self in order to meet patient needs.
