- As our Primary Care Nurse Practitioner, you will be assigned to a skilled nursing facility to treat geriatric patients for their primary care needs.
- You will be responsible for primary care for independent living, assisted living and skilled nursing residents at a clinic on the campus.
- The ideal candidate will maintain a strong relationship with the client facility, patients and family members.
- Excellent clinical and interpersonal skills are required as well as the desire to be a part of a team that functions at the highest level.
- Your duties will include collaboration with administration, nursing, therapy, and families as well as preventative care.
- The selected candidate will always put the patient and facility first.
- Extremely competitive hourly rate of $65.00 to $75.00.

- This is a dream position for the nurse who loves geriatrics. You will practice with a Nurse Practitioner in order to improve the health of an entire facility. You will be an integrated cornerstone for the facility and have the opportunity to lead programs in order to decrease falls, decrease hospitalizations and improve quality of life.
