Kindred Hospitals is Hiring a RN Case Manager - Dayshift (Mon-Fri) /Kindred Hospital Seattle First Hill Near Seattle, WA

We are looking for a full time RN Case Manager for our team in Seattle.

Kindred Hospital Seattle – First Hill is an 80-bed long-term acute care hospital, we offer the same critical care patients receive in a traditional hospital or intensive care unit, but for an extended recovery period. Our team plays a vital role in the recovery process for chronic, critically ill and medically complex patients. We are located in the heart of downtown Seattle, with easy access to I5 and close to Freeway Park at: 1334 Terry Avenue, Seattle, WA 98101.

Our Registered Nurse Case Manager provides oversight and intervention through assessing, planning, coordinating, monitoring and evaluation of services required to respond to individual patient needs. Responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a discharge plan in collaboration with the multidisciplinary team.

  • Responsible for utilization review, discharge planning and care coordination with the patient, patient’s family, physician and health care team.
  • Coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
  • Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family.
  • Coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition.
  • Documents professional recommendations, discharge plan, care coordination interventions, and case management activities to effectively communicate to all members of the health care team.

Qualifications

  • Associate Degree in Nursing or Bachelor’s Degree in Nursing, BSN-Preferred, OR LSW
  • Currently licensed as a Registered Nurse in the state of practice
  • 2 years experience in case management OR 3 years experience in clinical nursing
  • Certification in Case Management, Preferred
