As a, Registered Nurse Hospice Triage, you will: Facilitate the implementation of nursing interventions and appropriate preventative and therapeutic nursing procedures for after-hours needs Collaborate with after-hours staff members to provide continuous assessment of immediate needs, interpret findings, initiate appropriate action and monitor results Assist in the investigation and resolution of quality issues in conjunction with the Continuous Quality Improvement Committee Recognize and respond appropriately to PCG and after-hours staffs concerns Communicate effectively with all members of Interdisciplinary Team by obtaining report from PCMs and admissions staff prior to start of shift. Report after-hours assessment, interventions, death and admissions Attend nursing meetings as required Professionally identify self to patient/family/caregiver, physicians, nursing facility staff and other referral sources Effectively prioritize and process calls to provide optimal patient care Maintain confidentiality in all areas Gather appropriate information to determine needs and suitable course of action Relate complete and accurate information in verbal and written communication. Maintain clinical records, statistics, reports as required and in compliance with local, state and federal regulations Initiate and maintain effective communication with Patient Care Manager and General Manager as well as members of the Interdisciplinary Team Assure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures Coordinate on-call staffing assignments and scheduling of on-call visits. On-call visits will be coordinated with all Interdisciplinary Team members in order to effectively utilize staff and other resources As call volume dictates, make visits to patients to provide nursing support as needed for symptom management, emotional support, and education. As call volume dictates, complete a death pronouncement or admission visit
Bachelors degree. Registered Nurse licensed in the state. A.C.R.N.H. certification preferred. Minimum of one year of general nursing preferred, with six months to one year of experience in hospice or pain and symptom management interventions preferred. Ability to work within an interdisciplinary setting. Understanding of hospice philosophy, principles of death/dying, and the ability to advise the caregiver and company staff in proper management of symptoms Compliance with accepted professional standards and practices. Demonstrate excellent observation, assessment, nursing judgment and communication skills Ability to provide accurate written documentation in a timely manner. Self-directed with the ability to work with minimal guidance from the on-call administrator. Flexible and cooperative in fulfilling all obligations.
