Job Summary: As an Admissions RN of Kindful Health you will work with the Clinical Director to lead a care team including LPN(s), Aides, MSW and Chaplain to provide best in class care to a reasonably sized case load. The RNCM will present at IDT every 2 weeks with updates from the care team and coordinate with the Clinical Director and Medical Director to assure best in class care.

Essential Education and Experience: Must be a graduate of an approved school of professional nursing. Must have at least 2 years of nursing experience in the Hospice service line in the state of Texas.

Responsibilities & Duties: The Admissions RN will be responsible for:

  • Adheres to all regulatory requirements and professional standards
  • Follows policies and procedures consistent with recognized standards of professional practice
  • Identifies system problems or inefficiencies that may negatively affect patient care
  • Manage clinical excellence within your patient census.
  • Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores
  • Collaborate with your Clinical Director, Medical Director and entire team to provide a "concierge" level hospice experience

This leader is just as happy in the field providing patient care as they are growing the knowledge and capabilities of their fellow team members.

Compensation:

Salary - $90,000-95,000 base plus bonus opportunities.
