Job Description

Assess psychosocial status of patients related to patient's illness and environment and communicates finding to the registered nurse. Carries our social evaluations and plans intervention based on evaluation findings. Provide patient, caregiver and family counseling. Assists physician and other team members in understanding significant social and emotional factors related to health problems. 

Qualifications

Holds bachelors or masters degree in social work, demonstrates good verbal and written communications and organization skills.
