Job RequirementsResponsibilities:- Conduct initial assessments of patients to determine their physical, emotional, and spiritual needs- Develop comprehensive care plans in collaboration with the interdisciplinary team- Coordinate and monitor services provided to patients, including nursing care, medical treatments, and psychosocial support- Provide education and support to patients and families on end-of-life care options- Advocate for patients' rights and preferences in care decisions- Collaborate with healthcare providers, community resources, and other stakeholders to ensure continuity of care- Maintain accurate and timely documentation of patient assessments, care plans, and interventions
