Responsibilities :
- Offer consultation to staff and healthcare providers while initiating a personalized social work plan of care.
- Act as a liaison between our company and the community, facilitating seamless integration of services, coordinating care, and fostering community relationships through public relations activities.
Requirements :
Education : Master's degree in social work. Licensed Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii).
Basic Life Support certification required (American Heart Association).
- Experience : Minimum of one year in social work, preferably in a hospital or medical setting, with expertise in advanced care directives and insurance referrals.
- Skills : Strong communication skills, empathy, and a genuine passion for making a positive impact in the community.
Benefits :
- Contract Type : Permanent
- Work Arrangement : In-person
- Schedule : Day shift, 8 : 00 AM to 4 : 30 PM, with at least three shifts per month required on weekdays and weekends.
