* Kettering Health Dayton, formerly Grandview Medical Center, has been providing care to Dayton and surrounding communities for over 90 years.
* Over 20 specialties, including orthopedics, heart and vascular, surgery, and trauma services.
* KH Dayton expanded their Emergency Department as well as their heart & vascular services to include an additional catheterization lab and electrophysiology lab.
* Partners with Gem City Market to provide fresh fruits and vegetables every Tuesday within the hospital.
* One of the largest osteopathic teaching hospitals in the United States.

Responsibilities & Requirements
KEY JOB RESPONSIBILITIES:
* Optimize antimicrobial use in the hospital and network through prospective chart review with feedback
* Monitor key metrics for the Antimicrobial Stewardship Program (ASP) while serving as the antimicrobial drug information expert
* Participate in protocol and policy development and in hospital and/or network committees,
* Precept pharmacy students and/or residents
* Work closely with other members of the Department and Network Pharmacy to address any needs that arise
* Perform other duties as assigned
REQUIREMENTS:
Education:
* B.S. Pharmacy or PharmD degree from accredited College of Pharmacy
* Completion of a PGY1 Residency preferred
* Completion of a PGY2 Residency in Infectious Disease preferred
Work Experience:
Completion of ASHP-accredited PGY1 residency and an Antimicrobial Stewardship Certificate from either the Society of Infectious Diseases Pharmacists (SIDP) or Making A Difference in Infectious Diseases (MAD-ID)*
OR
3 years of pharmacy experience and an Antimicrobial Stewardship Certificate from either the Society of Infectious Diseases Pharmacists (SIDP) or Making A Difference in Infectious Diseases (MAD-ID)*
OR
Completion of ASHP-accredited PGY2 residency in Infectious Diseases Pharmacy
OR
Board of Pharmacy Specialties (BPS) Certification in Infectious Diseases Pharmacy (BCIDP)
* Must be enrolled within 2 years of hire date if pharmacist does not already have a certificate

ESSENTIAL FUNCTIONS:
* Perform prospective and retrospective antimicrobial review, feedback, and (when indicated) intervention.
* Participate in the development of infectious diseases-related protocols and order sets.
* Collaborate with other members of the antibiotic stewardship interdisciplinary team to establish models and procedures to audit and provide prescriber feedback to optimize antimicrobial prescribing practice.
* Represent antibiotic stewardship to front-line prescribers and administrative leaders.
* Actively contribute, and provide leadership in related committees, groups or initiatives relating to antimicrobial stewardship.
* Participate in the preparation of reports intended to document the impact of the program on quality, safety, and cost of antimicrobial use.
* Serve as medication information resource for caregivers, patients, and families.
* Lead didactic sessions on antimicrobial stewardship topics with medical residents and students upon request.
* Precept pharmacy students on rotation(s) which includes prioritizing and managing work assignments.
LICENSURE, CERTIFICATIONS, REGISTRATIONS:
Must register with the Ohio State Board of Pharmacy as a Pharmacist and maintain active status during their employment at Kettering Health
